Job Opportunity: SASH Coordinator at VSHA

The Vermont State Housing Authority is expanding its participation in the Support and Services at Home program, (SASH program) in the Chittenden/Franklin areas. VSHA needs a coordinator to build trusting relationships with participants in SASH in order to develop a thorough knowledge of each participant’s strengths and challenges as they pertain to remaining safely in his/her home. Specific roles and responsibilities of the SASH Coordinator fall into three broad categories of interventions: transitional care, coordinated care and self-management education. Bachelor Degree and experience in a related social service field or the equivalent combination of background and experience required, including training in the aging process. Position requires knowledge of community services, strong interpersonal skills and the ability to communicate and deal effectively one-on-one and in group settings. Coordinator will provide services in both Franklin and Chittenden counties with travel required. One year full time limited service position will be based in Milton.
