Housing & Community Service Specialist (3) Division of Disability and Aging Services Choices for Care/Money Follows the Person Seeking three Housing Specialists to join the Money Follows the Person (MFP) team. The role will be to provide individual support and technical assistance to the case management and local housing agencies for nursing home residents who wish to return to the community and are in need of housing. These are temporary positions that will be full time for 2013 and negotiable for the following years of the grant (2016). These are non-benefited positions; mileage reimbursement, phone and computer will be provided. The coverage of the state will be divided into three regions. Bachelor’s degree and four years’ experience in a human services program preferred. Consideration will be given to individuals with housing, case management, social work or service coordination experience.
